It's a well protected structure, not only because it is encased in bone, but also because blood vessels that serve the brain are lined by tightly- packed cells that make up the 'blood-brain barrier'. That security is there to protect brain tissue from dangerous pathogens and foreign molecules, but it's also a barrier to the entry of therapeutic agents. However, there could be a way to sneak drugs past those barriers by opening them transiently, explains Dr Matthew Campbell from Trinity College Dublin. He's part of a team pioneering an approach that uses specifically designed small interfering RNA to suppress production of a protein called claudin-5, a key component of the barrier that stops drugs moving across from the bloodstream. Injecting siRNA systemically has been shown to improve the permeability of blood cells around brain tumours called gliobastomas in pre-clinical models. "Given the life-threatening nature of this sort of condition, systemic administration of siRNA targeting claudin-5 to allow periodic drug access to tumours would be ethically justifiable," says Dr Campbell. The approach could also be used in the treatment of traumatic brain injury, where suppressing claudin with siRNA could open the blood-brain barrier and encourage the movement of water away from the injury site, thereby reducing the potentially lethal effects of swelling, he added. In a related project, the team has also developed an inducible system to suppress claudin-5 and open the barriers in blood vessels serving the retina. That offers options to treat degenerative eye diseases with systemic drugs rather than repeated injections into the eye.
